Registered Nurse
Position Summary:
Provides services to clients in accordance with the State Nurse Practice Act, Agency policy, and accepted professional standards of practice.
The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.
Qualifications:

Graduate of an accredited school of professional nursing. BSN, PHN preferred.

Current license as a Registered Nurse in the state(s) of practice.

Minimum of one (1) year experience in an acute care setting or equivalent experience.

Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.

Certified Home Health Aide
Position Summary:
Provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort.
Qualifications:

Successful completion of a formal certification training program and/or a written skills test and competency evaluation.

Be at least eighteen (18) years of age.

Minimum of six (6) months work experience in a supervised setting, preferably health care facility.

Demonstrated ability to read, write, and follow a written Plan of Care.

Good verbal, written, and interpersonal skills.

Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
Licensed Vocational Nurse
Position Summary:
Performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and Agency.
All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/LVN delivers care to clients as delegated by the RN Case Manager.
Qualifications:

Graduate of an accredited school of professional nursing.

Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.

Minimum of one (1) year experience in an acute care setting or equivalent experience.

Care coordinator
Position Summary:
Receives, evaluates, and coordinates referral activity.
Performs scheduling activities that facilitate Agency operations. These activities include coordination of employee schedules with client wants/needs, processing intake information and communication with referral sources.
Follows up on pending unresolved coverage issues.
Distributes information to appropriate division for scheduling of employees.
Assure documentation is complete, accurate, reflects care delivery and legal requirements.
Qualifications:

RN/LPN with current license to practice in state(s) of operation.

Minimum of one (1) year nursing experience.

Knowledge of community resources, Medicare and state regulations, ACHC, CHAP requirements, Joint Commission requirements, and reimbursement criteria for third party and private payers.

Knowledge of medical terminology or a general medical background desirable.

Medical Social Worker
Position Summary:
Provides services and/or establishes standards addressing the social and/or emotional factors that impact the clients ability to achieve the goals of the Plan of Care.
Services are provided under the direction of a physician and in conjunction with other health care team members in accordance with the established policies and practices of the Agency.
Qualifications:

Masters Degree from a school of Social Work accredited by the Council on Social Work Education.

Minimum of one (1) year's experience in a health care setting or equivalent experience. Home care experience preferred.

CPR certification, as applicable.

Demonstrates excellent written and verbal communication skills and strong interpersonal skills.

Demonstrates knowledge of resources available in the community.

Director of Patient Care services
Position Summary:
Responsible for direction of all home health clinical services.
Establishes, implements and evaluates goals for providing home health services that support the mission and philosophy of the Agency
Qualifications:

Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice.

Baccalaureate degree in nursing or equivalent experience, or related field or higher degree preferred with (3) three years of experience within the last (5) five years in a home health agency or other related field, at least one year of which was in a supervisory or administrative capacity or a registered nurse with (4) four years experience within the last five years in a home health agency or related field, at least (1) year of which was in a supervisory or administrative capacity.

Demonstrated ability to supervise and direct clinical and administrative personnel.

Knowledge of business management, State, Federal, Medicare and ACHC, CHAP, Joint Commission regulations, as applicable.

CPR certified.

Excellent written and verbal communication skills and strong interpersonal skills including the ability to market and relate tactfully with customers and other members of the community.

Physical therapist
Position Summary:
Organizes and conducts medically prescribed physical therapy programs to restore function, prevent disability following disease, injury or loss of a body part, and helps patients reach maximum performance levels. Plans therapy involving physical exercise, massage, heat, water, etc. Utilizes various mechanical and electrical equipment, and prosthetic and orthotic devices. Evaluates, records and reports on patient progress. We are looking for a PT who will do evaluations and discharges, follow up visits are done by experienced PTA's.
Requirements

Graduate of an accredited school of Physical Therapy

Knowledgeable of the American Physical Therapy Association Code of Ethics and the Guide to Physical Therapy Practice.

Excellent communication skills

Minimum of (2) year’s experience in the healthcare field. Home care experience is preferred.

Community Liaison
Position Summary:
Responsible for managing marketing program.
Establishes and maintains relationships with customers and referral sources.
Responds to requests and concerns. Negotiates contracts with third party payers, government agencies and other entities requesting services.
Qualifications:

Bachelor’s degree in business or related field preferred.

Previous experience in health care management or marketing, preferably in hospice care preferred.

Demonstrated knowledge of regulatory requirements and restrictions in home health services.
